How to Register
STEP 1:
Register your player online by following the link below to the Affinity Website. Please sign-in or create a new account (new players) to register your player(s). Fill out the player and parent information including uniform and hoodie sizes.STEP 2:
Read and check the boxes on the Electronic Legal Agreements (ELA) STEP 3:
Set up payment information and submit payment (note: your registration will not complete if you don't select and submit your payment type)STEP 4:
Print Forms: Receipt (includes the Player Participation Form) and ELA Forms.STEP 5:
Deliver the following to the Club Registrar
- 3 copies of your player's Birth Certificate or other proof of age document (see below)
- Signed Player Participation Form
- Signed ELA Form (needs player's signature also)
- A head shot photo of your player (this can be emailed or uploaded through your registration account also)
- Registration Fees (only if paying by check) or Club Copy of your Receipt

- Acceptable Forms of Proof of Age: Birth Certificate, Current Drivers License or State Issued ID, Passport, Alien Registration Card issued by the U.S. Government, Uniformed services Identification and Privilege Card (DD Form 1173) issued by the uniformed services of the United States, Birth registration issued by an appropriate government agency, Certificate issued by the Immigration and Naturalization Service attesting to age or a certification of an American Citizen born abroad issued by the appropriate government agency, Hospital, baptismal or religious certificates WILL NOT be accepted, Current Drivers License

Frequently Asked Questions about Registration
How much are the fees and what are they for?
Ages U-9 to U-14 fees are $237.00
This is for Fall 2011 and Spring 2012
The fee includes the UYSA state fee, uniform, hoodie, ball bag, field and ref expenses, equipment, and training expenses.
Ages U-15 to U-18 fees are $266.00
For Boys Teams this is for Fall 2011 only.
For Girls Teams this is for Spring 2012 only.
The fee includes the UYSA state fee, uniform with additional Home Game jersey, hoodie, ball bag, field and ref expenses, equipment, and training expenses.
When are Registration and Fees due?
Online Registration must be completed by June 15th. Registration Fees are due July 15th. Any fees paid after that will be charged a $10 late fee. The online registration system will allow you to pay by E-Check, Credit/Debit Card, and also set up a payment program if you wish.
How do I register?
Registration will be available online after May 18th at Nebo United Online Registration. You will need to:
- Register and pay for your player online by filling out the required information
- Upload head shot of the player (UYSA requires a new and recent picture each year)
- Print all ELA documents from Online Registration
- Make 3 copies of the players Birth Certificate
- Turn in Online Registration forms and Birth Certificates
